Cardinal Care: Virginia’s Unified Medicaid Program
On January 1, 2023, Virginia’s Medicaid system officially became Cardinal Care, a streamlined program designed to connect Medicaid members with essential healthcare and support services. Whether you were part of Medallion 4.0, CCC Plus, or FAMIS, you are now enrolled in a single system of care called Cardinal Care.
This change reduces program confusion, improves care coordination, and ensures consistent coverage as your needs evolve—all while continuing to offer the same trusted services.
What Is Medicaid Cardinal Care?
Medicaid Cardinal Care is Virginia’s new umbrella program for all Medicaid and fee-for-service health coverage. The system includes:
- Cardinal Care Managed Care (CCMC) – combining Medallion 4.0 and CCC Plus into one plan
- Cardinal Care Fee-for-Service – for those newly enrolled or with limited coverage
- Home and Community-Based Services (HCBS) Waivers – continuing under CCC Plus HCBS
The Virginia Cardinal Care program was developed to:
- Eliminate transitions between multiple Medicaid plans
- Improve access to care and services
- Ensure every eligible Virginian receives the right care at the right time
What Is Staying the Same?
Good news for members: nothing is being taken away.
✔ You keep your benefits
✔ You can continue seeing the same doctors
✔ You stay with your current Managed Care Organization (MCO)
✔ You still have one open enrollment opportunity per year
No re-enrollment is necessary if you're already covered. Your transition to Cardinal Care is automatic.
What’s New in Cardinal Care Medicaid?
Here’s what’s changing under the Cardinal Care Medicaid system:
- Members now receive a new ID card with the Cardinal Care logo
- The CCC Plus and Medallion 4.0 names are being phased out
- Open enrollment periods for some adults are now region-based
- All Medicaid members, including fee-for-service and managed care, are now part of one unified system

What Is the Difference Between Cardinal Care Medicaid and Cardinal Health Medicaid?
A common source of confusion is the term “Cardinal Health Medicaid.” While similar sounding, it refers to two different things:
- Cardinal Health is a national healthcare services company that supplies medical products and logistics—it does not administer Medicaid in Virginia.
- Cardinal Care Medicaid is Virginia’s official Medicaid program for healthcare and long-term support.
So if you’re in Virginia and looking for home health care through Medicaid, you need Cardinal Care—not Cardinal Health.
